    there's a sensible way to solve this problem:

    have everyone in america go for a physical. the attending physician will give each person a "statement of health" and ways for that person to achieve the best health possible. if that person decides to follow the health program, then they can be offered a standard or preferred health insurance rating, based on their statement of health. if that person declines to follow their statement of health, then they will be forced to self-insure. when people are forced to save to pay for their health care in the future, behaviors will change.

    sin taxes are stupid, but then again is the expectation that you should feel free to do whatever you want healthwise and expect other entities to pick up the tab (or to share in the tab).

    We do have some laws that prevent the ER from turning you away for not being able to pay. I've read about some laws that prevent a hospital evicting you if you run out of money during treatment.
